Case Manager

Location: Warsaw

Hours: Full-time and Part-time available. Flexibility to work two evenings.


Providing excellent customer service, the Case Manager will assist with the daily operations of the Outreach Team detailed below.

Education and Experience

  • Minimum HS diploma with two years of relevant experience. Associates degree preferred. Bilingual in English and Spanish.
  • Strong computer skills. 
  • Working knowledge of medical terminology. Valid NYS driver's license.

Responsibilities

  • A complete understanding of OOH mission and services.
  • Supports the Supervising Case Managers and Outreach team members on specific case loads.
  • Works with patients and community agencies to increase awareness of health promotion and disease prevention programs. Develop effective working relationships to reduce barriers in the community for at-risk patients.
  • Schedule/confirm appointments as needed.
  • Monitors documents, labs, referrals, appointment, and "jelly beans" directed to the department.
  • Handles incoming and outgoing phone calls.
  • Documents patients contact and follow up via the EMR
  • Provides written and verbal translation/interpretation as necessary.
  • Arranges transportation or transports referred patients as necessary.

Community Health Worker

Location: Warsaw and Hornell

Hours: Per diem

Now hiring per diem bilingual community health workers. We are looking for caring, community-minded individuals to join our Outreach Team in Warsaw and Hornell.  Various shifts from 8:00 am – 9:00 pm.  Our Community Health Workers function as part of an important support network for medically vulnerable patients, such as migrant farm workers and/or individuals with multiple chronic health conditions.  Every day will be different, but you can expect to schedule and confirm appointments, transport patients when necessary to their Oak Orchard Health or referral appointments, document patient and staff activities via Electronic Medical Record (EMR), monitor EMR messages directed to the Outreach Team, handle incoming and outgoing telephone calls, and provide written and verbal translation/interpretation for Spanish-speaking non-English-proficient patients.

The successful applicant will be fully bilingual in English and Spanish, have strong computer skills, good people skills, and at least a high school diploma or GED.  A working knowledge of medical terminology is helpful.  On the job training will be provided.  Valid NYS Driver License required. Field Community Health Worker

Join our award winning field crew providing health outreach services to the area's migrant and seasonal farm workers. This is a seasonal, per diem position.  Assist with set up and clean up of mobile medical unit equipment, register patients and record patient and staff activities in Electronic Medical Records, and provide written and verbal translation/interpretation for Spanish-speaking non-English-proficient patients and health care professionals including our physicians and public health nurses.

The successful applicant will be fully bilingual in English and Spanish, have strong computer skills, good people skills, and at least a high school diploma or GED.  A working knowledge of medical terminology would be helpful, but can be learned.  On the job training will be provided.  Valid NYS Drivers License required.
