Community Health Worker

Location: Warsaw and Hornell

Hours: Per diem

Now hiring per diem bilingual community health workers. We are looking for caring, community-minded individuals to join our Outreach Team in Warsaw and Hornell.  Various shifts from 8:00 am – 9:00 pm.  Our Community Health Workers function as part of an important support network for medically vulnerable patients, such as migrant farm workers and/or individuals with multiple chronic health conditions.  Every day will be different, but you can expect to schedule and confirm appointments, transport patients when necessary to their Oak Orchard Health or referral appointments, document patient and staff activities via Electronic Medical Record (EMR), monitor EMR messages directed to the Outreach Team, handle incoming and outgoing telephone calls, and provide written and verbal translation/interpretation for Spanish-speaking non-English-proficient patients.

The successful applicant will be fully bilingual in English and Spanish, have strong computer skills, good people skills, and at least a high school diploma or GED.  A working knowledge of medical terminology is helpful.  On the job training will be provided.  Valid NYS Driver License required.
