About Oak Orchard Health

Our History

For over four decades, Oak Orchard Health has provided comprehensive, culturally aware integrative health services to the agricultural workers and their families living and working in our community.  Our providers are uniquely qualified to deliver this high-quality care as they are fully aware of and understand the health and safety needs of these patients.  We offer health screenings for high blood pressure, high blood sugar, colorectal cancer and HIV. Also, we can assess simple complaints such as back pain, indigestion and athlete’s foot and provide the appropriate health care for these minor injuries and illnesses right in the field. Our patient engagement professionals can make the appropriate arrangements for these patients to get any necessary, more comprehensive medical care in our office locations.  Additionally, they will assist with translation, transportation, navigation of insurance and sliding scale payment plans.

Originally founded in 1966, Oak Orchard has grown from a migrant health project into an integrated health center with multiple locations providing health care services for everyone located in the communities we serve. Currently serving over 22,000 patients at six locations, Oak Orchard Health is a recognized patient-centered medical home and 501(c) nonprofit Federally Qualified Health Center (FQHC) located in the towns of Brockport, Albion, Lyndonville, Hornell and Warsaw. We also serve patients throughout Orleans County with our Mobile Dental Unit. PATIENT CENTERED MEDICAL HOME (pcmh)

OOH has been a certified Patient Centered Medical Home since 1973. Oak Orchard Health's certification as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA) means our patients can count on high quality health care delivered with an emphasis on care coordination and communication. What you can expect as a patient of OOH:

  • Comprehensive Care – Oak Orchard Health can meet the large majority of each of our patients' health care needs. Providing comprehensive care requires a team. That team includes physicians, advanced practice nurses, physician assistants, nurses, Nutritionists, educators, and care coordinators. At Oak Orchard Health, the goal of our Care Teams is to provide each patient with the best care uniquely suited to his or her specific needs.

  • Patient-Centered Care – Oak Orchard promotes wellness and prevention by encouraging and supporting our patients to work with their Care Team and to be active participants in their health care. We strive to build relationships with our patients … understanding and respecting each patient's unique needs, culture, values, and preferences.

  • Coordinated Care – Our Care Teams coordinate care across the broader health care system, including specialty care, hospitals, home health care, and community services and supports.

  • Increased Access to Care – Oak Orchard delivers accessible care with enhanced office hours. We offer evening and weekend hours and provide around-the-clock patient access to a member of his or her Care Team.

  • High Quality Care – We are committed to offering the highest quality of care to our patients. Oak Orchard’s Quality and Performance Improvement team is constantly engaging in performance measurement and improvement – measuring and responding to patient experiences and satisfaction – and practicing population health management, all in an effort to help the Care Teams provide the best care possible for their patients.


federally qualified health center (fqhc)

A Federally Qualified Health Center (FQHC) is a care center that is community-based and patient-directed. FQHCs promote community health by providing health care services to EVERYONE and improve access to health care to all, regardless of insurance status or ability to pay.

We provide equal access to quality health care to all persons regardless of age, sex, color, ethnicity, national origin, or the ability to pay.
